Medical Forms

The process for the medical forms has two steps.

First step
The athlete must first download the medical form at the following link: https://wtgf.org/wp-content/uploads/2024/04/TFWC2024-Medical-Form-updated-2-April-2024.pdf

The form must be brought to your doctor to obtain the requested information and laboratory results.

Second step
connect to the following link https://form.jotform.com/World_Transplant/transplant-football-world-cup  to transfer the information from the paper form to the online form.
